How to Request an Affidavit

Page Vault can provide an affidavit for any capture made with the Page Vault Browser, Site Crawler, Chrome Extension, or our Batch tool.

Our standard affidavit can be viewed here. Any requested changes are subject to review by our counsel and will be charged to the client at our prevailing hourly legal fees rate.

Steps for Requesting an Affidavit

Email support@page-vault.com with the following:

• The best billing contact/information

• The Matter ID to associate with the invoice (if applicable)

• A list of Capture IDs needed for the affidavit

  • Alternatively, export the captures as a .zip file and send to support@page-vault.com

• Whether a Case Caption should be included

  • If yes, attach the Case Caption in a Word document

Pricing and Delivery

• Standard affidavit: $500

  • Some subscriptions may include affidavits at no additional cost

• Notarization: Available for an additional fee

• Standard delivery: 5–7 business days

  • Expedited delivery is available for an additional fee

Once we receive your request, we’ll respond with a timeline and any follow-up questions. The completed affidavit will be delivered via email.
